Getting Started with the Assistive Devices Program (ADP)

Step 1: Get Your ADP Application. You can find it online

Step 2. Get a Medical Check Up. If it’s your first time getting forms signed for Lymphedema compression garments, the signing physician must be a specialist in one of the following areas:

  • Cardiovascular, Vascular, Orthopaedic, Plastic or General Surgery
  • Radiation or Medical Oncology
  • Internal medicine, Paediatrics, or Physiatry

N.B. Assuming applicable experience, General Practitioners in Oncology (GPO),  Gynecologic Oncologists, Wound Care Specialists or Dermatologists may also provide this assessment. 

​If you’ve been approved through ADP for compression garments previously, your forms need to be updated every two years and/or if there is a change in your medical condition, by a family doctor, nurse practitioner or specialist physician (as listed above).

Step 3. Visit an ADP Authorizer. Which Marie-Anne Hisem RMT,CLT is an approved Authorizer. Before signing your form, the ADP authorizer will identify the exact type of device you need and confirm your eligibility for ADP funding assistance. For lymphedema compression garments or sleeves you must get the authorization of one of the following healthcare professionals registered with the ADP: registered massage therapists, occupational therapists, physiotherapists, or registered nurses.

Step 4: See an ADP Registered Vendor. Your authorizer will refer you to an ADP registered vendor, where a certified fitter will measure you and order the device(s) that you require. The certified fitter will also provide education about the use and care of the device(s).
